Prototype/Engineering Delivery Confirmation Input Form

Instructions:

1. Complete the form inputs, where:
• "receivedPieces" is the number of pieces/services you have shipped/given to the customer
• "companyName" is the company of the customer (if there is no company, put customer name and surname)
• "customerRef" is the customer's first and last name respectively
• "email" is the customer's email
• "projectName" is the project name
• "sentDate" is the date we shipped the package / delivered the service
• "receivedProduct" is a description of the product or service.
  Write it as product1; product2... e.g. "10 prototypes of deviceXY; software code for LED;...".
• "HGResp" is the HG person filling up this form
2. Click on the Submit Button to get the Link
3. Click on the "Copy Link and Send Email" button to copy the link, Outlook will open
4. Send the copied link to the customer through your email
